Lists everything the carriers ask for before they will review a toll-free verification, in the order to present it, with what you have already supplied and where each item stands. Read it with no verification named to get the blank form before starting one, or name a verification to see its own answers. An item's state says where it stands, and this read emits five: not_supplied asks you for the value; supplied means a draft holds it; in_review means the carrier holds the whole dossier, so nothing can change; approved and rejected are the carrier's verdict on the dossier, shared by every supplied item, and reason carries its words on a refusal. The set is open, since review can gain stages; a state you do not recognise is not yours to act on.
