Build your first sign-up form
Get started with sign-up forms.
Learn how to create and customize effective sign-up forms for your website or Shopify store. This guide covers:
Creating and naming your form

Adding forms to your Shopify store or website
Head to Marketing > More > Sign-Up Forms on the sidebar.
Here, you can view all your previously created forms and check their status (e.g., active, draft, and pending).
Click Create new sign-up form in the top right corner.
To set up your form, simply:
Name your form: Give it a clear and recognizable name, e.g., "Bird.com multi-channel opt-ins."
Select your default language using the dropdown list.
Add a tag (optional): For example, "promotions" if you plan to send this list promotional content.
Select the list for subscribers to be added to. In this example, we will use Website signups.
If your form is for Shopify, select your store from the dropdown menu. If no stores appear, go back and install Shopify as an integration. Otherwise, select "any other website."
Click Create sign-up form, and you will be able to start designing your form.
Go to the Styles tab to choose how your form will appear on desktop. You can toggle between different views (e.g., modal, pop-over, embed). For pop-overs, you can select where on the screen the form will display.
Adjust form size by inputting pixel height and selecting from the background options (small, medium, or large).
Customize colors using sliders or by entering specific RGB, HSL, or hex codes to match your branding.
Adjust the background color, border color, and enable a background image if desired. You can also add spacing within the form to fit your brand style.
Set the timing: Decide when the form should display (e.g., immediately, on exit intent, or after a specific time). For this example, we’ve set the form to appear 2 minutes after the user enters the website.
Set the frequency: For example, show the form again five days after the visitor closes it.
Prevent showing the form again if it has already been submitted.
Navigate to the Targeting tab to select where you want your form displayed. You can choose to embed it on any domain or limit it to specific pages. For example, you can paste your Shopify store URL here. You can also choose to show the form on all paths or specific paths.
You can also choose to display the form to specific locations (e.g., show to UK visitors but exclude Netherlands visitors). Simply select the country, or countries, from the dropdown list.
Go to the Form Image section. You can add an image by uploading a file, providing a URL, or selecting a pre-saved image from your media library.
Select the image size from the dropdown, add your alt-text for accessibility, and choose the image position and alignment. The image size will need to be adjusted based on this placement.
Navigate to Steps > Content to start editing your headings and body text to align with your campaign message, e.g., "Sign up and get 10% off."
You can edit the text size and styling within the editor and set your margin sizes to fit your brand.
Click Add block in the top right of the form. You can select to add:
Text
Buttons
Disclosure text
Email opt-in and phone opt-in fields
For this example, we will add an email opt-in. Select the channel you want to subscribe the contact to and any styling you need by adding margins around your content.
Add a submit button
Click Add block and select the Button block.
Customize the text on your button (e.g., "Submit").
Choose the action for when someone clicks the button:
Submit the form
Close the pop-up
Navigate to a URL
If you select Submit form as the action, choose which contact list to add new subscribers to, and decide what happens next (whether the form closes or the user is taken to a new page). For example, you might want to direct users to the Bird homepage, so insert that URL into the target URL field.
Add a disclosure section
Add a disclosure section to your form by clicking Add block and then selecting Disclosure.
Edit the disclosure to comply with legal requirements, such as GDPR, by consulting your legal team.
You can add padding to your form by adjusting the margins above and below your disclosure text.
Customize the success message
After setting up the form, go to Success under the content step. Customize the message your customers will see after they submit the form, such as "Thank you for signing up" or a personalized thank-you message.
Now that your form is ready, simply click the Publish button to activate it.
If you're using Shopify, your form will automatically appear on your store right after publishing—there’s nothing else you need to do.
For other websites, you’ll find a code snippet after publishing. Copy this snippet, head over to your website’s settings, and paste it where you’d like the form to appear. Don’t forget
Open any of your sign-up forms to see your form data.
You can view how many people engaged with your form and how many submitted it.
To dive deeper into the data, use the dropdown menu in the top right corner to filter your results. You can view different time periods, see how many people viewed your form, check submission numbers, and compare different versions' performance.
And that’s it!
Your sign-up form is now ready to collect subscriptions. With targeted behavior, custom designs, and multi-language support, your forms can effectively engage your audience and boost conversions
