Build a short sequence around a single task: find an appointment, qualify a request or choose a product option. Ask only for information that changes the next step.

## Keep the form concise

Use relevant choices, sensible defaults and useful validation. Preserve essential meaning when translating labels and help text.

## Confirm the real reservation
